SnapCare Rewards Program Details

SnapCare Rewards is a rewards program offered by SnapMedTech, Inc., dba SnapCare for eligible SnapCare clinicians and healthcare professionals. For purposes of these rules and regulations governing Membership in the Program (hereinafter “Program Rules” or “Rules”), the terms “SnapCare”, “we,” “us,” and “our” refer to SnapCare, and the term “the Program” refers to the SnapCare Rewards Program. The terms “Member,” “Members,” “you,” and “your” apply to you as a Member of the Program; the term “account” refers to your SnapCare Rewards Membership account; and the term “snaps” refers to your SnapCare Rewards point value which has no cash value and may not be redeemed for a cash equivalent. The term “calendar year” means January 1-December 31 of each year. SnapCare reserves the right to amend, suspend, or change the Program and/or Program Rules at any time without notice. SnapCare reserves the right to end the Program with six months’ notice. [AT1] [RS2] Members do not acquire property rights in accrued snaps. By enrolling and/or participating in the SnapCare Rewards Program, you agree to abide by the Program Rules.

Reward redemption by the Member is subject to taxes, fees, and other government-imposed charges. The payment of any taxes, fees, and other government-imposed charges is the responsibility of the Member and must be paid at the time of redemption of snaps. SnapCare is not responsible for unauthorized access to a Member’s account and will not replace stolen snaps.

 

Membership

Membership in the SnapCare Rewards Program will be granted via personal invitation by SnapCare and is for individual clinicians only. Individuals must enroll separately and may not pool, combine or share snaps with other Members.

Member is limited to one account. A Member must provide their legal name matching their government issued photo identification, address, phone number, valid email address, gender, and date of birth as given to SnapCare for employment purposes. Account specific information will be sent via email to the primary email address in the Member’s SnapCare Rewards account.

Member is responsible for notifying SnapCare immediately of any change of name, email address, or physical address. Name change requests must be submitted in writing. Members may request a name change by contacting us via insert the proper info. The written request must contain the Member’s current contact information (name, address, telephone number, and email address if available), photocopies of legal documentation (ex. old government issued photo ID and new government issued photo ID, marriage license, divorce decree, etc.) and an informal letter indicating the new legal name. Email address and physical address changes may be made on SnapCare.com after logging into in your account.

Snaps that are not redeemed within 18 months of issue will expire. Should a Member close their account, the snaps in the account will be forfeited. All account activity and accounts in the SnapCare Rewards program are subject to review and adjustment by SnapCare at any time and without notice to the Member to ensure compliance with applicable rules. Any suspected fraud, misrepresentation, misuse, abuse or violation of applicable rules may result in cancellation of the Member’s account. If you are deactivated on the SnapCare platform or your account is marked DNU, any accrued Snaps are forfeited.

 

Earning SnapCare Rewards Snaps

Once enrolled in the Program, snaps can be earned by (a) joining an advertised local resource pool on the SnapCare platform as described in the advertisement; (b) working an assignment (first timecard approved) from the SnapCare job board or from a SnapCare recruiter. (c) any other published ‘call-to-action’ if taken in the required timeframe. Snaps will be awarded on the day earned and will be available in the SnapCare clinician’s ‘bank’ within 24 hours.

 

Redeeming SnapCare Rewards Snaps

Snaps can be redeemed for any items listed in the SnapCare Rewards catalogue as long as the snaps available in the account cover BOTH the item and shipping cost of the item. The number of snaps required to redeem a reward is subject to change at any time, however, the current SnapCare Rewards catalogue will be the trusted source, and the then-published snaps/shipping amounts will be honored at the time of redemption.

 

Additional Program Rules and Regulations

SnapCare may from time to time offer “bonus” snaps or promotional incentives for Members’ use. Such offers are not a part of the Program’s formal award structure and may be temporary.

By enrolling in the SnapCare Rewards Program you acknowledge that: (a) the Program is not mandatory in order to work at SnapCare; (b) all transactions and activities relating to the Program will be deemed to take place exclusively in the state of Georgia, United States of America, regardless of where any Member may be located or reside.; (c) all monetary amounts and fees relating to such transactions will only be expressed and processed in USD.

Any and all disputes arising out of or relating to the Program Rules and/or the subject matter hereof shall be governed by, construed, and enforced in accordance with the laws of the United States of America and, to the extent not preempted by Federal law, the laws of the State of Georgia without regard to conflict of law principles, regardless of the legal theory upon which such matter is asserted.

For purposes of the Program, you and we consent to the personal and exclusive jurisdiction and venue of the state and federal courts within Fulton County, Georgia. You and we also agree to litigate any disputes between or involving you and us relating to the Program exclusively in the state and federal courts within Fulton County, Georgia.

NO TRIAL BY JURY:

BY PARTICIPATING IN THIS PROGRAM, YOUWAIVE ANY RIGHT TO A TRIAL BY JURY IN ANY ACTION OR PROCEEDING BETWEEN YOU AND SNAPCARE ARISING OUT OF AND/OR RELATING TO THIS PROGRAM.

NO CLASS ACTIONS: YOU WAIVE ANY RIGHT TO PURSUE DISPUTES ON A CLASSWIDE BASIS. YOU AGREE THAT NO CLAIM OR DISPUTE ARISING OUT OF AND/OR RELATING TO THIS

PROGRAM MAY BE JOINED WITH A CLAIM OR DISPUTE OF ANY OTHER PERSON OR ENTITY, AND YOU MAY NOT ASSERT A CLAIM IN A REPRESENTATIVE CAPACITY ON BEHALF OF ANYONE ELSE IN ANY LAWSUIT OR OTHER LEGAL PROCEEDING.

The Program Rules, together with the Member’s enrollment, and the terms and conditions on SnapCare.com represent the entire, integrated agreement between the parties relating to the Program, and shall supersede all prior representations, understandings or agreements pertaining thereto, either oral or written. No other covenants, warranties, undertakings or understandings may be implied, in law or in equity.
