SnapCare offers various medical plan options to help you and your family live healthier lives. We offer both comprehensive and indemnity plans to provide for your needs, talk to your recruiter to sign up today!

Open Enrollment will be open the 1st – 19th of each month and go into effect on the 1st of the following month. Benefit elections must be made online through our new SnapCare EnrollVB Self-Service Portal

*Please note: while you may pair any of these plans together, you may only choose one of the MEC and/or Limited Indemnity Plans listed here (i.e. Virtual MEC + Base Limited Indemnity).

Telemedicine Plan

This plan, you and your immediate family members will have 24/7/365 unlimited virtual access to board-certified physicians through Teladoc. For a $0 consult fee, physicians can triage acute and common illnesses and write a prescription, if necessary, all through a virtual visit. These include cold and flu symptoms, rash, stomachache, eye/ear infection, UTI and many more. Additionally, under this plan, you will have access to a discounted cost for MRIs, prescriptions, hearing aids and more!

Virtual Minimum Essential Coverage (MEC) Plan

This plan includes access to preventive care services covered at 100%, unlimited telemedicine visits with a $0 consult fee, virtual behavioral health visits up to 3 times per year for a $50 copay, and copays for prescriptions under tiers 1 & 2. This plan is administered by SBMA and uses the PHCS network and does not offer any out-of-network benefits. This is not considered major medical.

Ultimate Minimum Essential Coverage (MEC) Plan

This plan includes access to preventive care services covered at 100%, offers copays for PCP, specialist, urgent care, Labs & X-Rays, unlimited telemedicine visits with a $0 consult fee, virtual behavioral health visits up to 3 times per year for a $50 copay, and copays for prescriptions under tiers 1-4. This plan is administered by SBMA and uses the PHCS network and does not offer any out-of-network benefits. This is not considered major medical.

Base Limited Indemnity Medical Plan 1

This plan offers various per diem benefits for emergency room and urgent care, hospital admission, confinement, inpatient & outpatient services, Lab, X-Rays and unlimited telemedicine visits. This plan is administered by Beazley and uses the First Health PPO network and does not offer any out-of-network benefits. This is not considered major medical.

Buy-up Limited Indemnity Medical Plan 2

This plan offers more enhanced per diem benefits for emergency room and urgent care, hospital admission, confinement, inpatient & outpatient services, Lab, X-Rays and unlimited telemedicine visits. This plan is administered by Beazley and uses the First Health PPO network and does not offer any out-of-network benefits. This is not considered major medical.

Vision

Comprehensive vision plan that includes annual eye exams and retail benefits administered by Reliance Standard.

Dental

SnapCare offers employees two dental plan options to choose from administered by Reliance Standard.
Both plans include in and out-of-network benefits for preventive, basic and major dental services.

Other Benefits

Life Benefits

SnapCare provides the opportunity to elect a Life benefit administered by Manhattan Life. You may elect to purchase Life coverage for you, your spouse and/or your dependent children. You must purchase Life coverage for yourself before you can elect for your spouse and/or dependents. Elections for the Life policy at a later date may require medical questions to be answered.

Disability Benefits

Employees may elect individual Short-Term Disability (iSTD) coverage with UNUM. It will be a guaranteed issue policy if you elect at the date of first eligibility. Elections for the iSTD policy at a later date may require medical questions to be answered.

Worksite Benefits

You may also choose to enroll in accident, hospital indemnity or critical illness benefits through Reliance Standard. These plans provide additional coverage for expenses that may not be covered by your medical plan.

Employee Assistance Program (EAP)

The WorkLifeMatters Employee Assistance Program (EAP) provides counseling and support services for you and your immediate family with confidential, professional counseling resources at no cost to you. This program includes unlimited 24/7 expert support. In addition to up to 3 face-to-face visits per issue per year.
